Making sure our residents and their family members feel at home at
Memory Haven is our highest priority. We encourage families to visit as
often as they like. While we consider Memory Haven to be the home for our
residents, we want family members of our residents to feel at home when

connected as possible to your loved one. That means you can call us at any
time of the day with questions. Our staff is always available.
Considering Memory Haven for Your Loved One?
We encourage you to visit Memory Haven for a tour. Seeing is believing. Our memory
care cottages and dedicated caregivers make Memory Haven truly unique in our
region. We’re proud of what we’ve built and the care we provide our residents and
look forward to meeting you.
After the tour, we encourage you to set up a face-to-face meeting with a member of
our cottage management team so that you have a chance to ask questions and we
can better understand how we might be of service to you and your loved one.
Getting Your Loved One Ready
Once the decision to move in is made, here is a general idea of
what happens next:
· Four Weeks Before Move-in - Plan what furniture you
will want to move into the space. If your loved one is
coming from another care facility, now is the time to give
a 30-day notice to that facility.
· Two Weeks Before - Our nurse will complete a detailed
needs assessment of your loved one to make sure his/her
individual needs are identified and addressed once
he/she moves in to Memory Haven.
· One Week Before - Locate pictures and write a brief
personal history of your loved one for his/her resident
· The Day Before - Move in bed and other furniture. Set up
the room with familiar items and label clothing.
Move-in Day - Arrive in the morning, bringing in medications.
Stay with your loved one through lunch. We’ll contact you
later in the day to share how your loved one is settling in.
We understand that moving your loved one to a new home is a
big decision. We encourage you to take time to prepare for the
change. We know that the separation from your loved one for the
first time can be stressful. We will do everything possible to
make the transition smooth and successful.
